Acute intracerebral hematoma measuring 4.1 x 2.7 x 3 cm (AP x TR x CC) noted in the right thalamo-capsulo-ganglionic region with surrounding edema. Expected interval evolution is noted with mild decreased density of the hematoma, compared to previous CT.

Mass effect noted on adjacent neuro-parenchyma in the form of effacement of regional cerebral sulcal spaces, right sylvian fissure with partial compression of right lateral ventricle and midline shift of ~ 2.5 mm to left side – No significant interval change seen in size of hematoma and its mass effect and midline shift seen.

Small area of gliosis is noted in the right high frontal lobe with gyral hyper density – unchanged as compared to previous CT

Small area of encephalomalacia is seen in the left frontal lobe with ex vacuo dilatation of the frontal horn of left lateral ventricle.

Ill-defined hypodensities noted in bilateral periventricular and fronto-parietal white matter regions – chronic ischemic changes.

Rest of the cerebral parenchyma shows normal attenuation.

No evidence of any obvious acute infarct.

Sella and parasellar regions are normal.

Cerebellar hemispheres are normal. Brainstem is normal.

Fourth ventricle is normal in size. CP angle is normal.

Visualized paranasal sinuses and orbits appear normal.
